This position requires travel to clients homes in the surrounding Metro counties.
The IHS Designated Coordinator is responsible for meeting with, setting up, and proving ongoing training and support to individuals accessing Sparkles’ Individualized Home Supports with Training (IHS) program (formerly ILS).
- Meet with clients and representatives to complete required paperwork, including, but not limited to, initial Intake/Annual Paperwork, Clinical Notes, Adult Protection Reports, Client Plans, and Client Discharges.
- Hire IHS Staff and onboard them.
- Ongoing client caseload management including establishing professional therapeutic relationships, diagnosis knowledge of assigned clients, maintenance of client files, and, when needed, providing client transportation according to established transportation policies.
- Communicate with assigned Client Representatives, IHS Manager, and/or IHS Supervisor of important client issues or concerns, including ongoing assessment of client goals and suggested changes to said client goals.
- Monitor each client’s authorized hours, ensuring client interactions stay within the allotted number of authorized hours and follow procedures for any needed changes to client hours.
- Prepare for, and attend, monthly supervisory meeting and/or periodic client plan reviews.
- Complete required trainings, as assigned.
- Provide services in a person-centered manner that aligns with the Mission and Vision of Sparkles.
- Work with leadership and other staff on the development of new processes and procedures.
- Maintain knowledge and understanding of DHS Home and Community Based Service Waivers, 245D Statutes, and best practices regarding service delivery.
Requirements:
- (1) A Bachelor degree in a field related to Human Services, and one year of full-time work experience providing Direct Care services to persons with disabilities or persons age 65 and older: or
- (2) An Associate degree in a field related to Human Services, and two years of full-time work experience providing Direct Care services to persons with disabilities or persons age 65 and older; or
- (3) A diploma in a field related to Human Services from an accredited postsecondary institution and three years of full-time work experience providing Direct Care services to persons with disabilities or persons age 65 and older; or
- (4) A minimum of 50 hours of education and training related to Human Services and disabilities; and four years of full-time work experience providing Direct Care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3)
- 1 year experience in a Human Service field
- Demonstrated creativity, critical thinking, and problem-solving skills
- Excellent organizational, problem solving, communication, and interpersonal skills
- Must pass a background study with the Minnesota Department of Human Services.
- Have a valid driver’s license, clean driving record and reliable transportation to use for work driving purposes.
- Ability to travel independently across the Sparkles geographic service areas.
