SERVICES

Sparkles Home Health Care is a licensed 245D provider of Night Supervision, Personal Support, Adult Companion, 24-hour emergency services, Homemaker, Individual Community Living Support, Respite, Individualized home support, Individualized home support with training and family Support. Trained Direct Support Staff members are available to help you with your needs when you want. Person Center planning is emphasized in our 245D programs. Staff are supervised and trained by our 245D coordinator

Individualized home supports (IHS) With Training

Individualized training support services develop, maintain, and improve the community living skills of an individual. In this program, we seek to Increase the individual’s independence by teaching skills so the person can perform tasks and activities with decreased dependence on caregivers. ILS training is direct training from a Direct Support staff to address the identified skill development needs of a person in the areas of:

  • Communication skills
  • Community living and mobility
  • Interpersonal skills
  • Reduction/elimination of challenging behavior
  • Self-care
  • Sensory/motor development involved in acquiring functional skills

IHS With Family Training

Services are provided to an individual and his/her family (including extended family members) in the family’s home and/or in the community to enable the person to remain at home. This includes training of the person and family members to increase their capabilities to care for and maintain the individual in the home. In-home family supports include assistance to develop, maintain or improve skills related to activities of daily living, such as:

Household chores (e.g., bed-making, washing dishes, laundry)

Eating and food preparation

Personal hygiene and cleanliness

Social and adaptive skills necessary to help the person live in the community

Health, safety and wellness

Household management.

IHS Without Training

Individualized Home Supports Without Training services provide support, assistance and supervision to adults or children who live in their own homes. A staff person provides direct supervision, cueing, guidance, instruction, assistance with activities of daily living, or assistance with coordinating and attending community living activities. Individualized Home Supports Without Training services can be provided in the person’s own home or community. It combines the previous services of Personal Support and Adult Companion

Individual Community Living Support (ICLS)

ICLS services offer assistance and support for older adults who need reminders, cues, intermittent/moderate supervision or physical assistance to remain in their own homes. ICLS is a bundled service that includes six service categories that includes.
  • Active cognitive support
  • Adaptive support service
  • Activities of daily living (ADLs)
  • Household management
  • Health, safety and wellness
  • Community engagement

Personal Support

Personal Support is non-medical care, assistance, and supervision provided in the individual’s home or community. This one-on-one service is intended to increase one’s independence and involvement with community activities.

Adult Companion

Adult Companion Services is non-medical care, supervision and socialization provided to individuals over the age of 18. Adult Companion Services help individuals gain more independence through skillful and therapeutic activities which helps to reduce isolation, learn to participate in the community, and work on coping skills to manage anxiety.

Night Supervision

Night Supervision Services is overnight assistance and monitoring provided by an awake staff in the individual’s own home. Examples of Night Supervision tasks include reinforcing independent living skills, monitoring for safety, or assisting with instrumental activities of daily living (IADLs).

Personal Supports

Personal Support is non-medical care, assistance, and supervision provided in the individual’s home or community. This one-on-one service is intended to increase one’s independence and involvement with community activities.

Respite Care

Respite is a short-term care service intended to provide a break, or relief, to an individual’s primary caregiver. Respite may be provided in the individuals, or the Respite maybe out of home. This service is available under various payment options including Medical Assistance Home and Community Based Services (waiver programs), and private pay.

Adult Companion

Adult Companion Services is non-medical care, supervision and socialization provided to individuals over the age of 18. Adult Companion Services help individuals gain more independence through skillful and therapeutic activities which helps to reduce isolation, learn to participate in the community, and work on coping skills to manage anxiety.

24 hours Emergency Assistance

Asking for help is a sign of strength. With our 24-hour service, you will be getting on-call counselling and problem solving or immediate responses for assistance at a person’s home due to health or personal emergency.

  3. Simple household repairs

Housing Stabilization Services (HSS)

Housing stabilization Service (HSS) is a home and community based services(HCBS) that support people with disabilities in finding housing; setting a budget; assisting individuals with knowledge and understanding the rules and regulations of lease. supporting individuals to create connections with landlords.

Purpose of HSS is to:

  1. Support individual’s transition into stable housing.
  2. Increase long term stability of housing in the community.
  3. Avoid frequent periods of homelessness or institutionalization.
